Thank you for completing our updated Heart Centre Biobank questionnaire! If willing, please fill out the full questionnaire to ensure we have the most complete and updated information.

All information will be collected on a secure SickKids server and responses are only accessible to approved Heart Centre Biobank staff. To complete the questionnaire you will be asked for a study ID # (e.g. BIO123456) that will allow us to connect the information to you/your family. An ID was provided at the bottom to the letter mailed to you. If you don’t know your study ID, please reach out to heartcentre.biobank@sickkids.ca with the participant’s name and a coordinator will be happy to provide you with one.
